About the Role

Care Synergy is seeking a compassionate and skilled Certified Nurse Assistant (CNA) to join our team as a Hospice Care Specialist. In this role, you will provide high-quality supportive and skilled care to patients in the hospice care, palliative care, and home care settings.

Key Responsibilities
  • Deliver care based on the Plan of Care (POC) established by the RN Case Manager and/or assigned RN.
  • Communicate with the Clinical Manager, RN Case Manager, or assigned RN regarding patient needs and preferences.
  • Participate in Interdisciplinary Team/Group (IDT/IDG) meetings and communicate with the Case Manager as needed.
  • Assist with ambulation, range-of-motion exercises, positioning, turning, and transfers according to patient abilities and preferences.
  • Take and record vital signs, observe changes in patient condition or behavior, and report any changes to licensed nursing personnel.
  • Provide personal care to patients, including bathing, peri-care, shampoo, oral hygiene, denture care, nail care, shaving, and assisting with dressing.
  • Aide/assist with patient toileting needs, including bedpan/urinal use, catheter care, changing briefs, and administering suppositories/enemas when delegated and supervised.
  • Address food and fluid issues by preparing meals, feeding patients, and/or offering fluids.
  • Maintain patient skin integrity to prevent pressure ulcers, skin tears, or other damage by changing incontinent patients, turning, repositioning, and applying lotion.
Requirements
  • High school diploma or equivalent.
  • 1 year CNA experience desired (hospice preferred).
  • Current Colorado CNA license.
  • Valid Colorado driver's license and proof of auto insurance.
  • Current CPR certification.
