Job Summary

The Differential Response Liaison is responsible for ensuring that Family Resource Center clients receive high-quality services and are case managed throughout the FRC and in the community as needed.

This position is concerned with the delivery of services in the community, requiring strong interpersonal skills to interact effectively with families at risk for abuse/neglect or out-of-home placement.

Key Responsibilities
  • Case manage families throughout the FRC and in the community
  • Deliver services in the community, including client interviewing, assessment, and service plan development
  • Work with families to identify and access community resources
  • Collaborate with FRC staff and community-based agencies to ensure seamless service delivery
  • Document outcomes and data per program protocols
Requirements
  • Minimum 21 years old
  • AA degree or equivalent work experience
  • 2+ years of experience in case management
  • Available regular work week and some weekends
  • Familiarity and experience with target populations
  • Familiarity and experience working in or with HSA, DCYF, and community-based agencies
  • Excellent organizational and communication skills
  • Conflict resolution and problem-solving skills
  • Proven ability to work with diverse populations
  • Strong written and verbal communication skills
  • Proficient with computers and Office 365
Preferred Qualifications
  • Bachelor's Degree in a related field or equivalent
  • Trauma Informed, Conflict Resolution
  • Knowledge of the Western Addition community
