Estimated Salary: $60,000 - $85,000 per year

About the Role:

We are seeking a skilled Transitional Care Specialist to join our team at Piedmont Healthcare. As a critical member of our healthcare team, you will be responsible for promoting patient and family wellness, improving care outcomes, and ensuring access to appropriate hospital and community resources.

Key Responsibilities:
  • Assess, evaluate, plan, and coordinate transitional care plans for patients with complex needs.
  • Advocate for services to support patients with psychosocial needs.
  • Conduct high-risk assessments within departmental and regulatory guidelines.
  • Coordinate daily with Care Managers to ensure seamless patient care.
Requirements:
  • Masters degree from an accredited social work program required.
  • No minimum experience required, but preference given to those with two or more years of experience in acute or post-acute settings.
  • Licensed Master Social Worker (LMSW) in the state of Georgia preferred.
