Job Summary: We are seeking a skilled and compassionate Home Health Nurse to join our team on a part-time basis. As a Home Health Nurse, you will work closely with our interdisciplinary team to provide personalized care to patients in their homes, promoting independence, dignity, and comfort.

Responsibilities:

  • Provide high-quality patient care, including assessments, care planning, and implementation of care plans
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
