Job Title: On Call Registered Nurse RN

Brighten Life Home Health is seeking a skilled and compassionate On Call Registered Nurse to join our team in St George. As a valued member of our Home Health team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Conduct thorough initial assessments of patients and their families to determine home care needs, including physical assessments and histories of current and previous illnesses.
  • Regularly re-evaluate patient nursing needs and make necessary revisions to the plan of care.
  • Develop and implement individualized care plans that establish goals and incorporate therapeutic, preventive, and rehabilitative nursing actions.
  • Administer medications and treatments as prescribed by physicians and provide health care instructions to patients and their families as needed.
  • Identify discharge planning needs and ensure a smooth transition for patients.
  • Participate in on-call duties and ensure that arrangements for equipment and other necessary items and services are made.
  • Supervise and evaluate home health aide care provided and prepare clinical notes and updates for primary physicians as required.
Qualifications:
  • Must be a Registered Nurse in good standing with the State of Utah and maintain a current license throughout employment.
  • Home Health experience is preferred, but not required.
  • Must be physically able to lift, turn, position, and transfer patients.
  • Have reliable means of transportation, current auto insurance, and a valid driver's license.
  • Demonstrate working knowledge of infection control principles and practices.
