Job Title: Weekend Home Care Staff Nurse

Harbor Health Services, Inc. is seeking a skilled Weekend Home Care Staff Nurse to join our Elder Service Plan team. As a key member of our team, you will provide high-quality nursing care to our geriatric clients in their homes.

Job Summary:

The Weekend Home Care Staff Nurse will be responsible for providing nursing care to our clients in their homes, including routine clinical duties, case management, and health teaching. You will work closely with our interdisciplinary team to develop and implement comprehensive care plans that meet the unique needs of each client.

Responsibilities:
  • Provide routine clinical nursing duties, including history taking, vital signs, administration of medications, and dressing changes.
  • Manage client cases, including communication with providers, center nursing staff, specialists, and other ESP staff members.
  • Collaborate with the Patient Care Team to develop and implement comprehensive care plans.
  • Triage patients regarding illness and medical therapy, and advise clients and caregivers on the most appropriate action.
  • Interact with clients and family members in a professional and compassionate manner.
  • Build trust and foster smooth relationships with clients and their families.
  • Promote patient confidentiality and right to privacy.
  • Establish and maintain an ongoing case management system, including contacting patients with abnormal lab results or who miss appointments.
  • Provide health teaching using appropriate information on health maintenance and disease management.
Requirements:
  • Associate Degree in Nursing, BSN preferred.
  • Current MA RN license and Basic Life Support (BLS) certification required.
  • Two (2) or more years of experience in a home care, professional, or business environment.
  • One year of experience working with geriatric patients.
  • Excellent communication and customer service skills.
  • Excellent critical thinking, problem-solving, and organizational skills.
  • Computer literacy (Microsoft Office applications with a focus on Word, Excel, and Outlook).
  • Experience with medical computer scheduling preferred.
  • Community health or ambulatory care experience preferred.
Work Schedule:

Friday evenings, Saturday, and Sunday (flexible as needed)

16 hours per weekend

Service area includes but is not limited to Boston neighborhoods, Brookline, Milton, Quincy, Braintree, Randolph, Stoughton (mileage/travel reimbursement provided)
