Job Summary:

We are seeking a skilled Physical Therapist Assistant to join our team. As a key member of our healthcare team, you will work closely with patients to evaluate, assess, and deliver services according to written plans of treatment established by physicians.

About the Role:

In this critical role, you will assist patients in achieving their goals and needs by developing personalized care plans using a variety of treatment techniques. You will provide quality, evidence-based care and promote healthy lifestyles by improving strength, flexibility, balance, and coordination.

Responsibilities:

  1. Evaluate patient progress and communicate effectively with healthcare teams to ensure seamless care delivery.
  2. Develop and implement individualized care plans that cater to each patient's unique needs and goals.
  3. Collaborate with patients, families, and caregivers to ensure a comprehensive understanding of care plans and progress.
  4. Conduct thorough assessments to identify areas for improvement and develop targeted interventions.
  5. Provide education and support to patients and their loved ones on maintaining independence and dignity.

Requirements:

  • Degree from an accredited Physical Therapist Assistant program.
  • 1 year of Medicare home health experience preferred.
  • Licensed/registered in the state of the HHA as a physical therapist assistant.
  • Proven work experience as a physical therapist assistant.
  • Ability to manage patients with different personalities and backgrounds.
  • Current knowledge of treatment practices and ongoing professional development.

Estimated Salary Range:

$45,000 - $65,000 per annum, depending on location and experience.
