About the Role

As a Transplant Healthcare Provider, you will coordinate and facilitate the delivery of care to patients within a multidisciplinary framework caring for pre and post kidney and pancreas transplant patients.

Job Description:
  • Become a member of a collaborative and supportive team-based service with several transplant nephrologists and advanced practice providers.
  • Work primarily in an inpatient setting at the M Health Fairview University of Minnesota Medical Center with some work in the clinic at the M Health Fairview Clinics and Surgery Center.
  • Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems and health care needs.
  • Practice independently and collaborate with physicians (as needed), and make appropriate referrals for evaluation or treatment.
Requirements:
  • A graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
  • Eligible or is certified through NCCPA, ANCC or AANP.
  • Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or licensed or eligible to acquire a license and prescriptive authority through MN Board of Nursing (NP's).
