Job Overview

Position: Chaplain/Spiritual/Pastoral Counselor

Role Summary:

The primary responsibility of this role is to deliver spiritual support to patients and their caregivers across all age demographics. This includes conducting spiritual evaluations, offering pastoral care, and facilitating referrals to address the needs of patients and their families.


Qualifications:

  • Education: Must be in good standing with a recognized religious denomination; a college degree is preferred, with a Master's in Divinity being highly desirable.
  • Licensure: A valid Texas Driver's License is required.
  • Experience: Previous involvement in a spiritual or religious organization is essential. A minimum of one year as a Chaplain in a healthcare setting is preferred.
  • Skills: Must possess a solid understanding of community spiritual resources and demonstrate strong interpersonal abilities.
  • Transportation: Reliable transportation and valid auto liability insurance are necessary.

Key Responsibilities:

  • Evaluate hospice patients and their families to determine spiritual needs and create tailored spiritual care plans. Collaborate with local clergy as necessary and assist in the development and revision of the comprehensive Plan of Care (POC).
  • Conduct prayer, worship, and ritual services for patients and families, providing diverse philosophical and belief options for discussion and sharing.
  • Offer individual counseling to patients and families, crisis intervention, and guidance on advance directives, funeral planning, and legal or financial responsibilities.
  • Engage with the Interdisciplinary Team (IDT) to contribute to the ongoing POC, documenting and assessing responses to interventions to ensure spiritual needs are met.
  • Collaborate with other staff to address spiritual, personal, and ethical concerns, and be available for bereavement or counseling support.
  • Utilize effective communication skills to assist family members in discharge planning and bereavement, involving local clergy as needed.
  • Complete necessary documentation promptly and assess patient and family satisfaction with care.
  • Make visits to patients and families prior to death and provide support during the bereavement period as required.
  • Fulfill mandatory continuing education requirements and participate in in-service training programs.
  • Promote the organization's philosophy and policies to ensure high-quality care delivery.
