BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in providing high-quality home health care services to our clients in the Lake Sunapee, NH area.

Job Summary:

We are looking for a compassionate and experienced Physical Therapist to work with our multidisciplinary health care team to provide rehabilitative care and skilled nursing to clients in their homes. As a Physical Therapist with BAYADA, you will have the opportunity to make a meaningful difference in the lives of our clients and their families.

Responsibilities:
  • Perform diagnostic tests and measurements to assess client needs and develop individualized care plans.
  • Develop and implement manual therapeutic exercises, gait training, balance, and other interventions to promote client independence and mobility.
  • Continually assess and revise care plans to ensure optimal client outcomes.
  • Educate and instruct clients, family members, or other client representatives on rehabilitative care and activities necessary for independent living.
  • Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Qualifications:
  • A current VT Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Solid computer skills and prior experience with electronic medical records (EMR) preferred.
