Job Summary

At BridgePoint Healthcare, we are seeking a dedicated and compassionate Registered Nurse to join our team. As a Registered Nurse, you will play a vital role in providing high-quality patient care and contributing to the overall success of our facility.

Key Responsibilities
  • Develop and implement individualized care plans in collaboration with residents, families, and interdisciplinary staff.
  • Provide direct and indirect patient care through the nursing process of assessment, planning, intervention, and evaluation.
  • Communicate effectively with physicians, nursing supervisors/managers, and colleagues to ensure seamless care transitions.
  • Participate in discharge planning and ensure a smooth transition of care for residents.
