GENERAL OVERVIEW:

The Speech-Language Pathologist operates as an autonomous healthcare provider dedicated to delivering tailored patient care that evaluates and addresses speech, language, social communication, cognitive communication, voice, fluency, and swallowing challenges in both children and adults upon physician referral. This professional is tasked with assessing referred patients, setting patient and family-centered objectives, and formulating and implementing treatment strategies aimed at enhancing or maximizing independence in the discharge environment.

KEY RESPONSIBILITIES:

  • Conduct initial assessments and follow-up evaluations of the patient's condition concerning speech, language, social and cognitive communication, voice, fluency, and/or swallowing in accordance with physician directives.
  • Perform comprehensive evaluations that include a thorough review of the patient's medical, surgical, and social history. Interpret relevant formal and informal assessments across various domains: Motor Speech, Verbal Expression, Written Expression, Voice and Resonance, Auditory Comprehension, Reading Comprehension, Hearing as it relates to speech and language, Swallowing function, Non-Verbal Communication, Orientation, Attention, Problem Solving and Reasoning, Memory, Oral Motor Function, Pragmatics, and Visual Perceptual Skills. Assessments may require adaptation due to medical complexities.
  • Develop personalized care plans that align therapist and patient/family goals to achieve realistic outcomes for prevention, intervention, and sustained wellness throughout the patient's life.
  • Implement and advance evidence-based therapeutic interventions in line with established and revised goals and care plans based on the patient's treatment response. Supervise support staff as appropriate.
  • Design and coordinate a comprehensive discharge strategy that includes recommendations tailored to the patient's specific needs in collaboration with other healthcare professionals.
  • Engage in peer consultations regarding patient care, and provide formal or informal mentorship to less experienced therapists.
  • Maintain educational qualifications by participating in and facilitating in-service training and departmental meetings regularly. Additional educational roles may include orienting new therapy staff, serving as a Clinical Instructor for students, and acting as a clinical fellowship supervisor.
  • Evaluate the educational needs of patients, address any barriers to understanding or learning, and involve the patient, family, and/or caregiver in the therapy process according to the patient's preferences. Collaborate and communicate with other healthcare team members to optimize therapy benefits and ensure quality care.
  • Complete necessary medical documentation, including initial assessments, follow-up evaluations, daily progress notes, care plans with associated certification dates, physician correspondence, insurance authorizations, and discharge summaries, ensuring compliance with professional, departmental, and payer standards.
  • Fulfill administrative responsibilities related to patient care, which may encompass scheduling support, monitoring patient attendance, ensuring accurate and timely billing practices, verifying appropriate referral and diagnostic coding, utilizing only medically necessary services, adhering to insurance authorizations, participating in peer reviews, and engaging in performance improvement initiatives.
  • Support the financial viability of the organization by meeting departmental productivity expectations.
  • Adhere to work schedule requirements based on site-specific needs and operational hours essential for patient care, which may necessitate flexibility in working at various locations.
  • Participate in community outreach and professional presentations.

QUALIFICATIONS:

To excel in this role, candidates must meet the following criteria:

A. Education / Training:

· Master's Degree, Certificate of Clinical Competence (ASHA) or completion of Clinical Fellowship Year (CFY) from ASHA.

B. Work Experience:

· Proven experience in evaluating and treating speech and language disorders in both adults and children, including conditions such as aphasia, apraxia, dysarthria, dysphagia, laryngectomy, stuttering, and voice disorders.

· Familiarity with head and neck cancer, tracheostomies, cognitive impairments, traumatic brain injury (TBI), dementia, and modified barium swallow assessments.

C. Certification, Licensure, Registration:

· Certificate of Clinical Competence from the American Speech-Language-Hearing Association (required) or completion of Clinical Fellowship Year (CFY) from the American Speech-Language-Hearing Association.

· Licensure in Speech Pathology in Michigan (required) or a temporary license during the CFY (required).
