About the Role

We are seeking an experienced Licensed Practical Nurse (LPN) to join our team at Apex Recovery LLC as a Care Coordinator.

Job Summary

This is a unique opportunity for a dedicated and compassionate LPN to make a meaningful impact in the lives of individuals struggling with addiction. As a Care Coordinator, you will play a vital role in providing high-quality patient care, ensuring seamless coordination of services, and promoting a supportive environment for recovery.

Key Responsibilities
  • Direct Patient Care: Provide individualized nursing care to clients, assessing their physical and behavioral health needs, developing personalized care plans, and maintaining accurate medical records.
  • Care Coordination: Collaborate with healthcare teams, family members, and community resources to ensure comprehensive care planning, timely interventions, and effective support systems.
  • Medication Management: Administer medications accurately, monitor client responses, and document medication-related activities.
  • Documentation and Reporting: Maintain thorough and up-to-date documentation of client progress, treatment plans, and any significant changes or incidents.
  • Communication and Teamwork: Foster open communication with clients, families, and healthcare professionals, promoting a collaborative and empathetic environment.
Requirements
  • Current LPN licensure and active registration in good standing.
  • Experience in social services, treatment addiction medicine, or related fields.
  • Valid state driver's license and reliable transportation.
  • Advanced computer skills, including proficiency in Salesforce, Microsoft, and Google applications.
  • Excellent interpersonal and communication skills, with the ability to interact effectively with diverse populations.
  • Self-motivated, organized, and able to work within established policies and procedures.
Working Conditions
  • Works in a fast-paced residential setting, interacting with clients, staff, and community partners.
  • May be required to lift up to 25 pounds and perform tasks that involve repetitive motion.
  • Occasional travel to meetings, training sessions, or conferences may be necessary.

Please note that we offer a competitive salary and benefits package, commensurate with experience and qualifications.

The estimated salary range for this position is $55,000 - $65,000 per annum, depending on qualifications and experience.
