- Compile and analyze data to identify trends and areas for process improvement, making recommendations to enhance patient care and experience.
- Provide education and support to patients and families on health-related topics, including disease management, medication adherence, and wellness strategies.
- Develop and maintain relationships with community resources and organizations to ensure access to necessary services and support for patients.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellence in patient care.
- Graduate of an accredited school of nursing with a current RN license in the state of employment.
- Minimum of three years of experience as a registered nurse in a healthcare setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to collect and analyze data to inform care decisions.
