Job Summary

We are seeking a highly skilled and compassionate Mental Health Worker to join our team at WES Health System. As a key member of our team, you will provide intensive care and support to children, adolescents, and their families in their homes, schools, and communities.

Key Responsibilities
  • Collaborate with therapists to provide individual, family, and group therapy sessions in various settings, using evidence-based treatment modalities.
  • Empower clients and families to identify and build upon their strengths, promoting resilience and self-sufficiency.
  • Maintain regular communication with all stakeholders involved in the client's care, ensuring a cohesive and effective treatment plan.
  • Provide emotional support, structure, and positive reinforcement to clients and families, helping them navigate challenging situations.
  • Assist in reducing the likelihood of hospitalization or residential treatment by providing timely interventions and support.
  • Evaluate client and family needs, interests, and response to treatment, making recommendations for adjustments as needed.
  • Connect families with community resources and supports, helping them access necessary services and benefits.
  • Follow up with clients requiring hospitalization, ensuring a smooth transition back to community-based care.
  • Intervene as needed to address problem behaviors, using counseling, de-escalation techniques, and other strategies to promote safety and well-being.
  • Execute discharge plans by making referrals to community resources and coordinating interagency meetings.
  • Obtain and update consents, ensuring compliance with regulatory requirements.
  • Make recommendations and referrals to other programs within and outside of WES Health System, as needed.
  • Complete and submit progress notes and required documentation in a timely, comprehensive, and legible manner.
  • Be flexible in scheduling appointments to meet client and family needs, prioritizing their well-being and convenience.
  • Contribute to quality improvement and outcome measurement tracking, providing data and insights to inform program development.
  • Meet productivity standards for the position, ensuring efficient use of time and resources.
  • Attend and complete all mandatory trainings, staying up-to-date on best practices and industry developments.
  • Participate in supervision, family meetings, staff meetings, and other team-building activities, fostering a collaborative and supportive work environment.
  • Adhere to WES Health System's Code of Ethics and comply with State Mental Health Code, upholding the highest standards of professionalism and integrity.
  • Participate in continuous quality assurance and program development, contributing to the growth and improvement of our services.
Requirements
  • Bachelor's Degree in a Human Services discipline, such as Psychology, Counseling, or Clinical Social Work.
  • One year of verified experience in case management or counseling with children and families.
  • Valid FBI clearance, criminal history check, and child abuse history clearance.
  • Participation in and completion of all CBH mandatory in-home services within three months of hire and annually thereafter.
  • Valid driver's license in good standing.
