Position Overview:
The Caregiver Support Specialist plays a crucial role in assisting family caregivers and their care recipients through a blend of empathy, technology, and established care methodologies. Working closely with state and case managers, the Caregiver Support Specialist offers essential support, education, and guidance to caregivers, equipping them with the necessary tools and confidence to provide superior care for their loved ones at home. Support and resources are delivered through technology, utilizing evidence-based practices that address both healthcare and social needs. This role typically involves managing more complex, higher-risk cases within the Coaching team, which may span multiple states and/or programs.

Key Responsibilities:
  • Engage, motivate, coach, and educate caregivers through phone and digital platforms; empower caregivers to manage their psychosocial, physical, and environmental health to enhance and sustain their wellbeing.
  • Oversee a caseload and utilize technology to facilitate effective coaching and information referral.
  • Administer payroll for the assigned caseload on a weekly basis.
  • Evaluate caregiver and care recipient needs, assisting caregivers in formulating and achieving care plan objectives; may involve referrals to external resources.
  • Collaborate with key stakeholders to assess care/coaching plans and caregiver engagement; identify challenges and propose solutions.
  • Document and report on coaching activities with a focus on quality and outcomes; provide recommendations as necessary.
  • Conduct case conferences, summary reports, and incident management in compliance with relevant state and program regulatory and/or accreditation standards.
  • Solicit feedback from caregivers to identify areas for additional support and process enhancement.
  • Administer payroll for your caseload on a weekly basis.
  • Perform other duties as assigned.
Qualifications:
  • Minimum of 2 years of experience in case management, particularly with elderly and disabled populations.
  • Exceptional active listening and communication skills, demonstrating strong empathy.
  • Proficient with technology and comfortable using (and teaching others to use) mobile applications.
  • Ability to multitask in a multi-system environment; proficiency in Microsoft Office is required.
  • Thrives in a dynamic, fast-paced environment.
  • Solid understanding and experience in care planning and case management is essential.
  • Multilingual abilities are highly desirable.
