We are seeking an experienced Home Health Registered Nurse (RN) to join our leadership team at Residential Home Health and Hospice. In this role, you will serve as a Home Health Registered Nurse Case Manager, overseeing patient care operations and collaborating with our interdisciplinary team to achieve exceptional outcomes.

About the Position:
This leadership role presents a unique opportunity for a seasoned RN to leverage their expertise and leadership skills to drive patient-centered care and improve operational efficiency. You will work closely with patients, families, and healthcare professionals to develop and implement comprehensive care plans, ensuring seamless transitions and optimal patient outcomes.

Key Responsibilities:
- Oversee patient care operations, ensuring compliance with regulatory requirements and best practices
- Collaborate with the interdisciplinary team to develop and implement evidence-based care strategies
- Conduct regular assessments of patient care quality and safety, implementing improvements as needed
- Communicate effectively with patients, families, and healthcare professionals to ensure transparency and accountability

Requirements:
- Current RN license in the state where you practice
- Three years of RN experience, with demonstrated leadership and management skills
- Ability to drive between patient locations, using company-provided transportation if needed
- Strong analytical and problem-solving skills, with a commitment to continuous learning and professional development

Salary and Benefits:
- Estimated annual salary range: $80,000 - $115,000, based on market conditions and productivity
