Key Responsibilities
- Assume full accountability for overseeing, guiding, and fostering safe clinical practices and high-quality patient care around the clock. Collaborate with healthcare administrators, medical professionals, nursing teams, and various other staff members to enhance clinical outcomes and care processes for all patients within the network.
- Leverage extensive knowledge of clinical and disease processes to drive improvements through various roles and partnerships. Ensure the development of clinical-ready programs, staff, and quality initiatives while adhering to all regulatory and professional standards.
- Promote the mission of healing and nurturing within the program, the organization, and the community. Report directly to the Senior Director of Quality.
Educational Development
- Create and implement educational resources for patients and their families regarding disease prevention, recognition, and post-discharge care. Advocate for policies and resources that facilitate optimal clinical care, ensuring access to necessary equipment and personnel.
- Develop protocols for post-discharge follow-up to monitor recovery and address any ongoing concerns, enhancing continuity of care and patient satisfaction.
- Collaborate with healthcare teams to streamline and coordinate care processes, ensuring timely and appropriate interventions for patients.
- Establish feedback mechanisms from patients and families to identify areas for improvement and implement changes that enhance satisfaction and outcomes.
Financial Management
- Oversee the budget for the clinical program, including allocations for staff training, equipment, educational materials, and quality initiatives. Monitor spending to ensure alignment with budgetary constraints and organizational objectives.
- Conduct cost-benefit analyses to assess the financial impact of prevention and management strategies, justifying resource allocation and identifying cost-saving opportunities.
- Stay updated on reimbursement policies and ensure compliance with documentation practices to maximize reimbursement and prevent revenue loss.
- Identify resource gaps in care delivery and recommend solutions to optimize utilization, prioritizing investments that significantly impact patient outcomes.
- Seek external funding opportunities through grants and partnerships to support research and quality improvement initiatives, ensuring compliance with grant requirements.
Program Design and Implementation
- Design and implement a clinical program that aligns with national guidelines and best practices. Collaborate with clinical teams to integrate protocols into workflows and standardize care approaches.
- Monitor adherence to clinical protocols, providing feedback and implementing necessary improvements. Establish systems for accurate data collection related to clinical cases.
- Analyze data to identify trends and opportunities for enhancing patient care.
Quality Improvement
- Develop evidence-based protocols for early recognition and management of conditions, ensuring alignment with current guidelines.
- Establish key performance indicators to track outcomes and implement systems for real-time data collection.
- Enhance patient safety through medication safety protocols and risk mitigation strategies. Conduct investigations to identify system failures and implement corrective actions.
- Facilitate audits and inspections related to clinical care, leading interdisciplinary teams focused on quality improvement.
Collaboration and Education
- Collaborate with healthcare professionals to establish program goals and objectives. Develop and deliver educational programs on prevention and evidence-based treatment.
- Conduct training sessions to ensure staff are knowledgeable about clinical protocols and guidelines. Foster collaboration among departments for a coordinated approach to management.
- Communicate regularly with leadership and stakeholders to provide updates on program progress and outcomes.
Qualifications
- Experience: A minimum of 5 years in a clinical setting with a focus on quality improvement.
- Education: Bachelor's Degree required.
- Certification/Licensure: RN license.
- Knowledge/Skills/Abilities: Performance Improvement, Strategic Thinking, & Change Management.
