Job Title: Admissions Registered Nurse

At Interim HealthCare of the Twin Cities, we are seeking a skilled and compassionate Admissions Registered Nurse to join our team. As a key member of our Home Care team, you will play a vital role in providing high-quality patient care and ensuring a seamless transition for our patients.

Job Summary:

The Admissions Registered Nurse is responsible for conducting admissions and visits for our Home Care team. This role requires a strong clinical background, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Conduct admissions and visits for Home Care patients
  • Assess patient needs and develop individualized care plans
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Provide education and support to patients and families
  • Document patient information and progress
Requirements:
  • Graduate of an accredited school of nursing and licensed to practice as a Registered Nurse in the state of Minnesota
  • Minimum of 12 months experience working in an acute care or home health environment
  • Evidence of current CPR certification
  • Experience in Home Care, including OASIS, preferred
  • Able to travel within a geographic area serviced by the office
