Job Description

As a LTSS Service Coordinator at Elevance Health, you will play a vital role in conducting service coordination functions for individuals in specialized programs. Your expertise will be essential in facilitating Person-Centered Planning, documenting preferences, needs, and goals, and managing physical and behavioral health needs.

Key Responsibilities:

  • Perform face-to-face program assessments using various tools with pre-defined questions, applying motivational interviewing techniques for evaluations, coordination, and management of individual waiver needs.
  • Identify members with potential clinical healthcare needs, addressing gaps in care, and coordinate cases with the clinical healthcare management team.
  • Manage non-clinical needs of members with chronic illnesses, co-morbidities, and disabilities, ensuring cost-effective and efficient utilization of long-term services and supports.
  • Document short and long-term service and support goals in collaboration with the member's chosen care team.
  • Identify members that would benefit from alternative levels of service or other waiver programs.
  • Participate in process improvement initiatives, serving as a mentor, subject matter expert, or preceptor for new staff.
