Position Objective:

The Community Support Team (CST) is responsible for providing a range of services including case management, crisis intervention, skills development, timely documentation, individual counseling, medication oversight, attending necessary meetings, quality improvement, and other essential tasks to support individuals with serious mental health challenges.

The Team Leader oversees the management and daily operations of a clinical team. Responsibilities encompass managing a caseload, facilitating group sessions, serving as the primary contact for inquiries and problem resolution, completing documentation within set deadlines, delivering training, ensuring quality assurance, supervising staff, handling administrative tasks, and participating in required meetings.

Qualifications/Essential Requirements:

  • Master's degree in social work, sociology, psychology, counseling, or a related field, or RN with a minimum of one year of mental health experience. QMHP certification is required.
  • Experience in supervising a team within a social services environment is preferred.
  • Proven ability to motivate and resolve issues with staff.
  • Experience working with individuals from diverse backgrounds and personalities.
  • Proficient in crisis intervention techniques to mitigate the effects of escalating symptoms on clients and their caregivers.
  • Ability to tailor verbal and written communication to suit the client and their family’s language style.
  • Competence in assessing and reassessing diagnoses, functional limitations, and formulating plans to address these limitations.
  • Skilled in coordinating services from various community resources.
  • Proficient keyboarding skills and familiarity with Microsoft Office Suite, with a capacity to quickly learn new software.
  • Exhibits a positive demeanor, empathy, and calmness.
  • Possession of a valid driver's license.

Key Responsibilities:

  1. Adhere to prescribed work hours and any additional hours deemed necessary for assigned responsibilities.
  2. Utilize available data to monitor team behavior in compliance with established standards.
  3. Provide constructive feedback on staff performance through team meetings, monthly supervision, and annual evaluations, with more frequent individual feedback as needed.
  4. Actively participate as a member of the management team, offering support to the team and other departmental staff.
  5. Complete all required monthly and quarterly reports within designated timelines.
  6. Engage in the on-call system as necessary.
  7. Ensure all clinical documentation meets regulatory requirements and organizational policies.
  8. Employ a variety of treatment concepts, evidence-based practices, and service modalities in care delivery.
  9. Demonstrate the ability to identify individuals in crisis, minimize the impact of the crisis on the individual and their support system, and determine appropriate next steps.
  10. Exhibit leadership by demonstrating self-motivation, seeking performance improvement, and problem-solving while addressing the needs of staff and clients.
  11. Maintain confidentiality in accordance with the professional Code of Conduct and legal frameworks.
  12. Deliver exceptional customer service consistently to all clients.
  13. Serve as a role model and demonstrate positive relations in representing the organization.
  14. Assume additional responsibilities as delegated by management.
