At Hospice of Franklin County, we are seeking a compassionate and skilled Hospice Registered Nurse to join our team. As a Hospice RN, you will play a vital role in providing high-quality care to patients and their families, promoting comfort, dignity, and quality of life.
Job Summary:The Hospice RN is responsible for coordinating and administering hospice care services to patients and families, ensuring that their physical, emotional, and spiritual needs are met. This includes conducting initial assessments, developing and implementing individualized plans of care, and collaborating with the interdisciplinary team to provide comprehensive care.
Key Responsibilities:- Conduct thorough initial assessments of patients and families to determine their needs and develop individualized plans of care.
- Coordinate and administer hospice care services, including medication management, symptom control, and emotional support.
- Collaborate with the interdisciplinary team, including physicians, social workers, and chaplains, to ensure comprehensive care.
- Provide education and guidance to patients and families on end-of-life care, pain management, and symptom control.
- Document patient care and progress, ensuring accurate and timely reporting.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
To be successful in this role, you will need:
- A minimum of one to two years of experience in healthcare, preferably in hospice or home health.
- Current Massachusetts licensure as a Registered Nurse.
- CPR certification.
- Valid driver's license.
- Preferred: Hospice and Palliative Care Nurse Certification.
