Job Summary

We are seeking a highly skilled and compassionate LPN Medication Care Manager to join our team at Sunrise Senior Living. As a key member of our care team, you will be responsible for providing high-quality care and services to our residents, with a focus on medication administration and management.

Responsibilities
  • Administer medications and treatments in a safe and organized manner
  • Conduct Service and Health Updates as directed by the Resident Care Director
  • Participate in the development of Individualized Service Plans (ISPs) and monthly updates
  • Transcribe orders and maintain accurate records
  • Collaborate with the care team to ensure seamless care delivery
  • Provide education and support to residents and families on medication management and other care-related topics
Requirements
  • LPN, LVN, or state/province specific licensed nurse credential
  • Previous experience working with seniors preferred
  • Ability to make choices and decisions and act in the resident's best interest
  • Ability to react and remain calm in difficult situations
  • Ability to handle multiple priorities
  • Possess written and verbal skills for effective communication and level of understanding
