Job Summary
Arc Broward is seeking a highly skilled Intake Coordinator/Social Services Specialist to join our team. As a key member of our person-centered team, you will be responsible for coordinating and facilitating the development and implementation of person-centered plans for individuals with developmental disabilities.
Key Responsibilities
- Actively recruit and facilitate the admission of consumers into the Adult Day Program and Group Homes.
- Coordinate and facilitate in the development and implementation of person-centered plans.
- Facilitate each consumer's access to entitlements, benefits, natural supports, and available community resources.
- Ensure compliance with local, state, and federal laws and agency regulatory standards and practice.
- Serve as a liaison with family members, governmental agency representatives, and other agency personnel.
- Act as a point of contact for families/participants with regard to services offered by the agency.
Requirements
- Bachelor's Degree in the Social Services field.
- 1-3 years related work experience with individuals with developmental disabilities.
