Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Hospice Nurse. As a member of our team, you will have the opportunity to make a meaningful impact on the lives of patients and their families, providing much-needed care in the comfort of their own homes.

Responsibilities
  • Perform initial nursing assessments on new admissions
  • Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness
  • Regularly communicate patient progress to the clinical manager, physician, and care team
  • Educate families on patient condition, needs, and care
  • Make referrals to other hospice care disciplines as needed, or document rationale for not doing so
  • Perform on-call responsibilities and on-call services to patients/families as assigned
Qualifications
  • Current RN license, specific to the state(s) you are assigned to work
  • One+ year of nursing experience required
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
