Job Overview
We are seeking an experienced Primary Care Physician to join our team at the Honolulu, Hawaii location. As a Primary Care Physician with the Department of Veteran Affairs, you will have the opportunity to work in a dynamic and supportive environment, providing comprehensive outpatient care to a panel of adult patients.
Key Responsibilities
- Provide Comprehensive Patient Care
You will be responsible for providing comprehensive primary care to a panel of assigned patients using the Patient Aligned Care Team (PACT) model of care delivery. This will involve conducting routine check-ups, diagnosing and treating illnesses, and developing treatment plans tailored to each patient's needs.
Interface with Other Healthcare ProvidersYou will work closely with other healthcare providers, including non-VA providers, to ensure smooth transitions in care and provide the best possible outcomes for our patients.
Achieve Performance Measure TargetsYou will contribute to achieving performance measure clinical outcome targets in the areas of access to care, quality of care, efficiency, process improvement, and patient satisfaction.
Prescribe Medications and Enter ConsultsYou will prescribe medications and enter consults to specialty clinics utilizing electronic medical records systems.
Practice Professional StandardsYou will practice in accordance with professional standards, core values, and a commitment to excellent service.
Complete Clinical Reminders and Progress NotesYou will complete clinical reminders for chronic disease management and prevention when due, as well as progress notes, encounters, secure messages, and view alert notifications according to VHA timeliness standards.
Work Schedule
The standard work schedule is 7:30am to 4:00pm, Monday through Friday, with occasional Saturdays on a shared rotating basis.
Estimated Salary Range
$200,000 - $250,000 per year, based on experience and qualifications.
