The Geri PACT Physician is a vital part of the Geriatrics Rehabilitation and Extended Care team at the VA Pacific Islands Healthcare System. As a member of this team, you will provide outstanding clinical and interpersonal skills to deliver patient-centered care to our nation’s heroes.
Key Responsibilities
- Provide outpatient primary and geriatric consultation services to Veterans.
- Collaborate with the Geri PACT team to establish a plan of care for Veterans.
- Participate in interdisciplinary team meetings to review active cases.
- Implement strategic care plans to facilitate utilization and access to other needed medical services.
- Monitor quality and service metrics to ensure departmental goals are achieved.
Requirements
- Excellent clinical and interpersonal skills.
- Ability to work collaboratively with the Geri PACT team.
- Strong communication and documentation skills.
- Adherence to confidentiality and mandatory training.
