Job Overview

Position: Case Manager

Work Schedule: Flexible hours, including evenings and weekends; on-call responsibilities required.

Key Responsibilities:

The Case Manager will report directly to the local manager and is expected to adhere to the policies and procedures of Axess Family Services while striving to uphold high service standards and professional practices. Each Case Manager will manage a caseload of up to 25 veteran residents, assisting them in achieving permanent housing.

  1. Conduct comprehensive assessments of the needs of veteran residents and/or their households, developing tailored support plans aimed at securing or maintaining permanent housing.
  2. Provide a broad spectrum of practical, personal, social, and general care services to veteran residents and/or their households.
  3. Assist veteran residents and/or their households in creating realistic and actionable housing plans that emphasize housing stability, incorporating measurable steps and timelines.
  4. Monitor, review, and document the agreed-upon support plans, adjusting as necessary to meet the evolving needs of the veteran residents and/or their households.
  5. Encourage active participation from veteran residents and/or their households in the decision-making processes regarding their services.
  6. Facilitate communication and collaboration with various agencies, including the Department of Veterans Affairs, Veterans Services Commission, and local housing authorities.
  7. Accompany veteran residents and/or their households to appointments with relevant agencies as needed.
  8. Maintain regular contact with veteran residents and/or households, providing support through childcare assessments, crisis intervention, referrals, and ongoing life skills development.
  9. Ensure that the housing provided to veteran residents and/or their households meets established organizational standards for habitability.
  10. Collect, update, and manage required data for the Homeless Management Information System (HMIS).
  11. Support Social Service Workers (SSSW) in their duties during their absence or as required.
  12. Provide staff support during on-call hours.
  13. Assist in covering shifts and addressing call-offs as needed.
  14. Engage in community outreach efforts to connect with homeless and at-risk veterans.
  15. Promote an inclusive environment, ensuring that no resident faces discrimination based on race, class, gender, religious beliefs, disability, or sexual orientation.
  16. Adapt to various working environments, whether as part of a team or independently.
  17. Implement Housing First, Trauma-Informed Care, and Harm Reduction principles, utilizing Motivational Interviewing and De-escalation Techniques as appropriate.
  18. Attend meetings, training sessions, and conferences as requested.
  19. Participate in community events that promote services for veterans.
  20. Foster positive working relationships with colleagues.
  21. Assist SSSW and other staff in maintaining up-to-date knowledge of fire regulations, emergency procedures, and health and safety standards.
  22. Comply with agency policies, COA regulations, and federal and state requirements.
  23. Maintain accurate records and logs.
  24. Perform other duties as assigned.
Qualifications:

Licensure: Valid Ohio Driver's License and ability to meet agency driving insurability requirements.

Background Check: Must pass a criminal background check (Ohio Bureau of Criminal Identification & Investigation).

Education: Bachelor's degree or currently pursuing a degree in social work or a related field preferred; high school diploma or equivalent required.

Experience: Preferred experience in homelessness, domestic violence, chemical dependency, and mental health; experience working with veterans is also preferred.
