The Role

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team at The Landing at Stone Oak, a luxury memory care community in San Antonio, Texas. As an LVN, you will provide nursing care to patients in accordance with established standards and practices, under the guidance and supervision of a licensed nurse.

Key Responsibilities

  • Collect and analyze pertinent health-related information to inform patient care.
  • Implement service plans formulated by the Director of Health Services to ensure high-quality patient outcomes.
  • Utilize safe and appropriate techniques and nursing skills to provide patient care.
  • Report changes in patient status to supervisors, including hospitalizations, discharges, and physician appointments.
  • Provide accurate and timely documentation consistent with the service plan/plan of care.
  • Communicate effectively with other disciplines involved in patient care, including weekends.
  • Administer medication per physician orders and observe for any adverse reactions.
  • Assist patients in learning appropriate self-care activities.
  • Organize and monitor human and material resources necessary for meeting patient needs.
  • Participate in staff development activities to enhance skills and knowledge.

Requirements

  • Graduation from an accredited program for a Licensed Practical Nurse or Licensed Vocational Nurse (LPN or LVN) and current licensure in the State of Texas.
  • At least one year of nursing experience in a healthcare setting.
  • Experience in geriatric nursing, emergency care, and first aid.
  • Valid Continuing Education credits as required by the State.
  • Desirable to have training in the special needs of cognitively impaired residents, including Alzheimer's and dementia care.
  • Ability to cope with the mental and emotional stress of the position.
  • Ability to work effectively with physicians, management, employees, and other outside contacts.
  • Ability to make independent decisions when circumstances warrant such action.
  • Leadership ability and willingness to work with and supervise staff.
