We are seeking a highly skilled and experienced Dietitian to join our team at Contra Costa County, CA. As a Dietitian, you will play a critical role in providing nutrition education and care to our patients and clients.
Key Responsibilities- Compile relevant information from various sources to make accurate nutritional assessments of high-risk patients.
- Observe and interview patients and family members to gather nutritional information.
- Develop and implement plans for the provision of optimal nutrition care, including menu planning and nutrition education.
- Develop educational materials and provide nutrition education and consultation to individuals and groups.
- Identify and prioritize patients requiring nutrition intervention.
- Monitor and record data in the nutrition care plan and charts the nutritional progress of individuals.
- Participate in interdisciplinary nutrition support rounds and update and participate in the development of nutrition policies and procedures.
- Provide in-service reviews and training to Nutritional Services staff and other healthcare team members.
- Current Certificate of Registration with the Commission on Dietetic Registration, the credentialing for the Academy of Nutrition and Dietetics.
- Successful completion of an Accredited Dietetic Internship Program and proof of acceptance for admission to the next scheduled examination for Registered Dietitian by the Academy of Nutrition and Dietetics.
- At least 4 years of experience in compiling relevant information from various sources to make nutritional assessments.
- At least 4 years of experience in developing and implementing plans for the provision of optimal nutrition care.
- At least 4 years of experience in developing educational materials and providing nutrition education and consultation to individuals and groups.
- At least 4 years of experience in observing and interviewing patients and family members for nutritional information.
This is a full-time position that requires working in a fast-paced environment with a high level of accuracy and attention to detail. The successful candidate will be required to work on weekends and on an on-call basis.
