Physical Therapist

Centrix Health Resources is seeking a skilled Physical Therapist to join our multidisciplinary team at Sunrise Health Services. As a Home Health Physical Therapist, you will play a vital role in rehabilitating and educating patients to regain and retain their independence in their home setting.
Key Responsibilities- Assess patients' home environment for safety hazards and architectural barriers, identifying equipment needs.
- Establish goals and a plan of care designed to improve patients' functional status, revising as necessary.
- Perform patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and record and evaluate findings to aid in establishing or revising specifics of treatment programs.
- Administer the exercise program utilizing specialized therapeutic exercise techniques and equipment.
- Plan and administer prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts.
- Recommends prosthetic/orthotic devices for patients or evaluates their devices and makes minor adjustments or recommends modifications by the prosthetist/orthotist.
- Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
- Perform and submit required documentation of patients' evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.
- Exercise necessary cost control measures.
- Strive to maintain positive internal and external customer service relationships.
- Demonstrate effective leadership skills.
- Demonstrate effective communication skills by conveying necessary information accurately, listening effectively, and asking questions when clarification is needed.
- Be dependable and arrive to work on time, taking breaks in expected time frames.
- Meet all productivity requirements.
- Demonstrate team behavior and promote a team-oriented environment.
- Represent the organization professionally at all times.
- Demonstrate initiative and strive to continually improve processes and relationships.
Must be a graduate from an accredited Physical Therapy program and licensed by the state of Arizona.
