Job Summary

We are seeking a skilled Physical Therapist Assistant to join our team at Professional Case Management. As a Physical Therapist Assistant, you will provide physical therapy services and education to clients in their homes, following a physician's Plan of Care.

Key Responsibilities
  • Provide physical therapy services and education to clients in their homes, following a physician's Plan of Care.
  • Complete visits in a timely manner and in accordance with Agency policy.
  • Document all services provided, observations, client progress, and recommendations on designated forms and submit to the agency within seven (7) days of providing service.
  • Report changes/concerns or schedule changes to the therapist and/or the nursing supervisor on the day of the observed change.
  • Participate in interdisciplinary activities per Agency guidelines.
  • Attend client care conferences as needed to provide information to other members of the team.
  • Promote referrals to other disciplines as appropriate.
  • Attend staff meetings and continuing education programs.
  • Participate in the Agency quality improvement activities pertinent to the role of Physical Therapy Assistant.
  • Promote personal safety and a safe environment for clients and coworkers.
  • Demonstrate knowledge of safety and infection control practices by compliance with Agency policies and procedures.
  • Recognize and report potentially unsafe situations.
  • Demonstrate safe practice in the use of equipment.
  • Assess environment and initiate actions to prevent accidents and promote safety.
  • Maintain confidentiality in all aspects of the job.
Requirements
  • Current license, registration or certification to practice as a Physical Therapy Assistant in the state(s) of employment or be qualified to take the physical therapy examination.
  • Graduate of an accredited physical therapy assistant program approved by the American Physical Therapy Association.
  • Minimum of six (6) months experience as a Physical Therapy Assistant.
  • Demonstrated written, verbal communication skills, and interpersonal skills.
  • Licensed driver with clean driving record and insured in accordance with state and/or Agency requirements.
Benefits
  • Standard Rate: $36.00 Hourly plus shift differentials, where applicable.
