We are seeking a highly skilled Care Manager LPN/RN to join our team at Sante. As a Care Manager, you will play a vital role in coordinating the delivery of services to our guests, working collaboratively with our facility's team members to ensure exceptional care and support.
Key Responsibilities- Monitor and document the effectiveness of treatment provided, coordinating the admission process and determining with the interdisciplinary team (IDT) appropriate post-discharge services.
- Coordinate and update Care Plans, attending meetings as requested by guests or deemed necessary by the IDT.
- Participate in daily IDT meetings, providing clinical reviews as needed.
- Complete assigned sections of Discharge Plan Instructions, reviewing clinical post-discharge instructions with guests upon discharge.
- A minimum of a Nursing Degree from an accredited college or university, or a graduate of an approved LPN/LVN program.
- A current, unencumbered, active license to practice as an RN or LPN/LVN through the State Board of Nursing.
- A minimum of two (2) years' experience in a hospital, nursing care facility, or other related healthcare facility.
