We are seeking a highly skilled and compassionate Senior Primary Care Physician to join our team at CenterWell Senior Primary Care. As a key member of our care team, you will be responsible for providing high-quality, patient-centered care to seniors in our community.

Responsibilities
  • Evaluate and treat patients in accordance with standards of care, utilizing your expertise in family medicine, internal medicine, or geriatric medicine.
  • Collaborate with our interdisciplinary care team to coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
  • Act as a medical expert and participate in daily huddles to ensure seamless care coordination and communication.
  • Help set a tone of cooperation and professionalism in practice by displaying a positive and approachable demeanor.
  • Complete medical record documentation in a timely manner, working with a quality-based coder to optimize coding specificity.
  • Follow policy and protocol defined by Clinical Leadership and participate in quality improvement initiatives.
  • Meet with Regional Medical Director to discuss quality of care, review outcome data, and address policy and procedure issues.
  • Participate in potential growth opportunities for new or existing services within the Center.
  • Participate in the local primary care 'on-call' program as needed.
  • Ensure personal compliance with licensing, certification, and accrediting bodies.
Requirements
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
  • Graduate of an accredited MD or DO program.
  • Excellent verbal and written communication skills.
  • Demonstrate a high level of skill in interpersonal relationships and communications with colleagues and patients.
  • Fully engaged in the concept of 'Integrated team-based care' model.
  • Willingness and ability to learn and adapt to practice in a value-based care setting.
  • Superior patient/customer service skills.
  • Basic computer skills, including email and EMR.
Preferred Qualifications
  • Board Certification or Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Bilingual English/Spanish is preferred.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • Medicaid Provider Number.
  • Minimum of two to five years directly applicable experience.
  • Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
  • Knowledge of Medicare guidelines and coverage.
  • Knowledge of HEDIS quality indicators.
