About the Role

At Thresholds, we are committed to providing exceptional care to individuals in their communities. As a Registered Nurse - Community Outreach, you will play a vital role in delivering medical and psychiatric support to those who need it most.

Key Responsibilities
  • Provide medical and psychiatric support to clients in their communities
  • Develop and implement treatment plans to meet the unique needs of each client
  • Administer medication, schedule appointments, and coordinate care
  • Remove barriers to healthcare through home visits and community outreach
  • Attend to medically related emergencies or crises, providing intervention as appropriate
  • Log medical care and track client progress
Requirements
  • Registered Nurse license
  • Associate's, Bachelor's or Master's degree from an accredited school of Nursing
  • Valid driver's license, current car insurance, and daily access to your own car
  • Willingness to provide transportation to clients
  • Willingness to participate in an on-call rotation
What We Offer
  • Competitive pay - Base Salary: $83,000, subject to increase with education and experience
  • 1st shift schedule: Monday-Friday 8:30am-5:00pm
