Job Summary

As a Physical Therapist Assistant Home Care, you will provide rehabilitation therapy procedures to patients under the direction and supervision of a physical therapist. Your primary responsibility will be to assist patients/caregivers in achieving their rehabilitation goals.

Key Responsibilities
  • Develop and implement treatment programs to restore and return maximal function to patients.
  • Assess the effectiveness of treatment and modify goals/treatment methods as needed.
  • Document treatment of each patient in compliance with department standards, medical-legal, and reimbursement procedures.
  • Instruct and train other rehabilitation staff, nursing, and medical personnel on patient needs and rehabilitation services.
  • Participate in ongoing process improvement, patient satisfaction initiatives, program development, and marketing activities.
Qualifications
  • Associate's Degree from an accredited physical therapy assistant program.
  • LIC-Physical Therapist Assistant - State of Michigan upon hire required.
  • CRT-Basic Life Support (BLS) - AHA/ARC within 90 days required.
  • LIC-Driver's License - State of Michigan (Home Care Business Lines only) upon hire required.
