Job Summary

This position is responsible for coordinating and implementing case management services for residents of Capuchin Apartments. The Case Manager will work cooperatively with the Lead Recovery Support Specialist to provide quality, inclusive supportive services to residents.

Key Responsibilities:

  • Create and review tenant supportive service plans and programmatic policies and procedures.
  • Oversee and observe assessments of tenants to determine eligibility for placement.
  • Provide case management services to Shelter Plus Care program participants and ensure all charts and records are documented and preserved according to Guest House/Milwaukee County and HUD standards.
  • Ensure all relevant documentation relating to the operation of the program is maintained and updated as needed.
  • Develop and maintain professional relationships with tenants as defined through the supportive services agreement.
  • Provide ongoing resources and referrals for clients.
  • Establish and maintain positive community relationships.

Requirements:

  • Knowledge and experience of utilizing community services and resources.
  • Ability to communicate clearly, both verbal and written.
  • Ability to plan, implement, and follow through on projects relating to the position.
  • Flexibility in schedule (some evening and weekend hours required).
  • Bachelor's Degree in Social Work, Human Services, or a related field preferred.
  • Experience working with individuals who have experienced homelessness, addiction, and mental illness preferred.
  • Crisis Prevention and Intervention training.
  • Computer proficient – working knowledge of Microsoft programs.
