Role Objective: The Healthcare Quality Manager will oversee the quality assurance program at Families Together Community Health Center (FTOC). This role is crucial in ensuring that the clinic adheres to all quality regulations established by governing bodies and that patient care outcomes are consistently monitored and enhanced.
Key Responsibilities:
- Develop and execute a robust quality management framework for FTOC.
- Evaluate and ensure compliance with quality and regulatory benchmarks.
- Identify areas for enhancement and formulate strategic action plans.
- Provide ongoing training to clinical personnel on quality management practices.
- Conduct systematic audits and evaluations of clinical processes and results.
- Collaborate with leadership to meet quality metrics and ensure high standards of patient care.
- Manage the Population Value Based Care grant and its components.
- Support the care coordination team in achieving their objectives.
- Facilitate the clinic's journey towards Patient Centered Medical Home (PCMH) certification.
- Act as the primary liaison for regulatory and accreditation entities.
- Ensure compliance of FTOC's policies and procedures with relevant regulations.
- Train internal stakeholders to ensure adherence to established policies and procedures.
- Monitor patient satisfaction and implement initiatives to enhance patient experiences.
- Maintain comprehensive records of quality management activities and outcomes.
- Build and sustain relationships with external quality improvement organizations.
- Lead quality improvement initiatives and projects.
- Ensure the electronic health record system supports quality assessment and enhancement.
- Work with the risk management team to identify and mitigate patient safety risks.
- Analyze quality data and prepare reports to communicate insights to clinical staff and leadership.
- Participate in interdisciplinary committees to advocate for quality and patient safety.
- Align the quality management program with the organization's strategic objectives.
- Conduct monthly quality board meetings to present quality metrics and develop action plans.
- Coordinate a quality workgroup to address improvement opportunities across FTOC.
- Stay informed on healthcare quality management trends and communicate updates to staff.
- Foster a culture of continuous improvement and patient-centered care within the clinic.
- Perform additional duties as assigned by the Director of Quality & Risk Management.
Qualifications and Skills:
- Bachelor's degree in healthcare management or a related discipline.
- At least 5 years of experience in healthcare quality management.
- Understanding of quality management principles and methodologies.
- Familiarity with regulatory and accreditation standards for health centers.
- Strong communication and interpersonal skills.
- Excellent analytical and problem-solving capabilities.
- Ability to collaborate effectively with diverse clinical and administrative teams.
- Proficiency in Microsoft Office and quality management software.
Additional Skills and Qualifications:
- Exceptional customer service skills.
- Professional verbal and written communication abilities.
- Capability to effectively communicate with senior management.
- Ability to create summarized reports and presentations.
- Willingness to undergo drug testing if required.
