Responsibilities:
  • Provide direct psychosocial intervention to residents and their families.

  • Coordinate resident visits with outside services, including dental and optical care.
  • Attend and document resident council meetings.
  • Support residents' families in coping with skilled nursing placement, physical illness, and disabilities.
  • Work with patients, families, and team members to plan discharge.
  • Conduct in-service programs to educate staff on psychosocial issues and patient rights.
  • Supervise and guide Social Services Assistants.
Requirements:
  • Bachelor's degree in Social Work (BSSW) or Master's degree in Social Work (MSSW) preferred.
  • Licensure as required by the state of employment.
  • One year of experience in a long-term care environment.
