Job Title: Registered Nurse Case Manager

We are seeking a skilled Registered Nurse Case Manager to join our team at Waterview Hills Rehabilitation & Healthcare. As a Registered Nurse Case Manager, you will play a vital role in providing high-quality care to our residents.

Job Summary:

The Registered Nurse Case Manager will be responsible for utilizing the nursing process to provide care to designated residents, administering medications and treatments, and documenting appropriately. You will also be responsible for reporting findings of ongoing observations and assessments to the designated Charge Nurse, Nursing Supervisor, and/or Unit Manager.

Responsibilities:
  • Utilize the nursing process to provide care to designated residents
  • Administer medications and treatments to residents
  • Document appropriately on assigned nursing unit
  • Report findings of ongoing observations and assessments to designated Charge Nurse, Nursing Supervisor, and/or Unit Manager
  • Demonstrate knowledge of basic test/lab collection values and deliver direct resident care consistent with policies/procedures and Resident Bill of Rights
  • Identify and respond timely to patient concerns/needs
  • Demonstrate effective interpersonal skills necessary to ensure appropriate quality patient care
  • Prepare resident assignments considering category of needs of patient and competency of personnel
Requirements:
  • Graduate of an approved school of nursing
  • Licensed and registered in good standing in New York State as a Registered Nurse
  • Experience with geriatric patients is desirable but not required
  • Sincere desire to work with the elderly, chronically ill, and disabled
  • Possess a spirit of cooperation and enthusiasm to create an atmosphere conducive to rehabilitation and growth
  • Knowledge of medical, social, and psychological needs of residents, general nursing theory and practice, and infection prevention principles/methods
  • Ability to communicate well verbally with residents, families, and staff
