About the Role

We are seeking a skilled and compassionate Registered Nurse to join our team at CenterWell Home Health. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in the comfort of their own homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients with diverse needs and conditions, including admission, case management, and follow-up visits.
  • Collaborate with physicians and other healthcare professionals to develop and implement individualized care plans that promote optimal patient outcomes.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them, ensuring seamless care transitions and coordination.
  • Communicate effectively with patients, families, and caregivers to educate them on care plans, medication management, and disease prevention.
  • Document patient progress, identify areas for improvement, and make recommendations for enhanced care delivery.
  • Participate in interdisciplinary team meetings to share knowledge, best practices, and patient outcomes, driving continuous quality improvement.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or a related setting
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
