Job Title: Community Health Worker - Bilingual Support

Join our team at Kalihi Palama Health Center as a Community Health Worker - Bilingual Support. In this role, you will provide case management, outreach, health education, and interpretation services to patients. You will also assist in training students and healthcare professionals on cultural issues and healthcare.

Key Responsibilities:
  • Provide interpretation services, including oral and written interpretation, health education, and counseling as needed.
  • Assist patients in filling out intake and other forms for medical visits.
  • Connect patients with entitlements, such as My Connection referrals, medical insurance, financial, and housing assistance.
  • Refer patients to outside agencies as needed.
  • Document all contacts in patient charts.
  • Attend training and classes to improve services.
  • Help patients connect with staff for medication refills.
  • Provide health education and counseling to patients.
  • Assist in training students and healthcare professionals on cultural issues and healthcare.
Requirements:
  • Fluency in English and Chinese/Vietnamese required.
  • Ability to read, understand, and translate technical/medical instructions in both English and the specified language.
  • Basic knowledge of computer skills.
  • High School Diploma or GED required.
  • Certification as a professional translator or community health worker preferred but not required.
Working Conditions:

The work environment is moderate, with reasonable accommodations made for individuals with disabilities. The noise level is usually moderate.

Work hours are 40 hours per week during clinic hours, 8:00 am - 5:00 pm, Monday through Saturday.

The physical demands of the job include regular talking and hearing, frequent sitting, and occasional walking, standing, and reaching.

The employee must occasionally lift and/or move up to 25 pounds.

The employee must exhibit sound and accurate judgment, support and explain reasoning for decisions, display original thinking and creativity, meet challenges with resourcefulness, and generate suggestions for improving work.

The employee must demonstrate the ability to solve problems practically.
