The RN Case Coordinator plays a crucial role in our organization by ensuring that patients transition safely from the hospital, identifying potential obstacles, and collaborating with the patient and care team to address these challenges.

Role Summary:

Evaluates, formulates, implements, and assesses patients' discharge planning requirements in conjunction with an interdisciplinary team.

Key Responsibilities and Qualifications:

PRIMARY DUTIES

  • Ensures adherence to discharge planning regulations.
  • Identifies and escalates issues related to inpatient and observation length of stay.
  • Evaluates patients at elevated risk for readmission, identifies underlying causes, and collaborates with the multidisciplinary team to develop discharge plans.
  • Engages with team members to identify opportunities for enhancing standard practices.
  • May also oversee utilization management, emergency department case management, and serve as a liaison with post-acute care providers.
  • Applies evidence-based practices and scientific knowledge to nursing care, ensuring that services are tailored to the patient's age and clinical needs.
  • Demonstrates professional nursing standards as outlined in the practice model, utilizing the ANA Code of Ethics to guide responses to evolving healthcare needs.
  • Maintains a vigilant and safe working environment.
  • Performs additional duties as assigned.

EDUCATIONAL REQUIREMENTS

  • Graduate of an accredited nursing program or equivalent educational background for licensure.

EXPERIENCE REQUIREMENTS

  • Minimum of three years of experience in acute care settings.

PHYSICAL DEMANDS

  • Regular lifting and carrying of objects weighing up to 25 lbs.
  • Frequent sitting, standing, walking, and performing repetitive movements.
  • Visual acuity for close and distant tasks, including color differentiation.
  • Effective communication skills, including the ability to hear alarms and other critical sounds.
  • Frequent use of keyboard and data entry.
  • Occasional bending, stooping, and lifting of heavier objects.
  • Rare climbing activities.
