You will work collaboratively with other team members to ensure patients receive exceptional service and care, and are connected to a Primary Medical Home.

Key Responsibilities:
  • Make personalized, outbound calls to patients to engage them in healthcare and schedule appointments.
  • Make reminder appointment calls and contact patients who have missed scheduled appointments.
  • Answer telephone calls in a professional and timely manner and respond to patients who leave messages.
  • Work cooperatively with all members of the care team to support the vision and mission of the organization, delivering excellent customer service.
  • Prepare and distribute mailings to patients.
  • Assist in preparation of daily and monthly reports.
  • Work independently or within a team environment.
  • Effectively work with other healthcare team members to ensure coordination of care.
Requirements:
  • High school graduate or G.E.D.
  • Excellent organizational skills.
  • Excellent communication skills and phone etiquette.
  • 1+ years customer office experience or patient experience.
  • Experience with an electronic medical record; Intermediate computer proficiency (multi-task through multiple applications including Microsoft Outlook, Excel, and Word).
