Job Summary:The Managed Care Patient Navigator is responsible for making contact with patients assigned to our health center, utilizing a variety of methods to engage them in healthcare and schedule appointments. You will work collaboratively with other team members to ensure patients receive exceptional service and care, and are connected to a Primary Medical Home.
Key Responsibilities:- Make personalized, outbound calls to patients to engage them in healthcare and schedule appointments.
- Make reminder appointment calls and contact patients who have missed scheduled appointments.
- Answer telephone calls in a professional and timely manner and respond to patients who leave messages.
- Work cooperatively with all members of the care team to support the vision and mission of the organization, delivering excellent customer service.
- Prepare and distribute mailings to patients.
- Assist in preparation of daily and monthly reports.
- Work independently or within a team environment.
- Effectively work with other healthcare team members to ensure coordination of care.
- High school graduate or G.E.D.
- Excellent organizational skills.
- Excellent communication skills and phone etiquette.
- 1+ years customer office experience or patient experience.
- Experience with an electronic medical record; Intermediate computer proficiency (multi-task through multiple applications including Microsoft Outlook, Excel, and Word).
