The primary purpose of this job position is to coordinate delivery of services to managed care and Medicare residents in collaboration with the facility's team member. This includes monitoring and documenting the cost-effectiveness of treatment provided, facilitating and coordinating the admission and discharge process, serving as the resident and family advocate, and acting as a liaison to insurance and medical management professionals.

Key Responsibilities

• Travel to assigned facilities as required
• Educate new and current facility staff on established Medicare and Medicaid policies and procedures
• Perform routine audits of facilities for compliance with established Medicare and Medicaid policies and procedures, rules, and regulations
• Educate facility staff on Medicare and Medicaid utilization following guidelines
• Ensure quality and appropriateness of resident care meets or exceeds company and industry standards
• Promote optimal communication among program staff members, facility staff, referral sources, physicians, and residents

Requirements

• Graduate of an accredited school of nursing
• Computer skills in Excel, Word, and websites
• Extensive travel and some overnight stays required
