As a Community Support Facilitator, you will work closely with our team to develop and implement individualized support plans, ensuring that each person receives the care and support they need to thrive.

Key Responsibilities:

  • Develop and implement individualized support plans for individuals with developmental disabilities
  • Provide direct support and advocacy for individuals with developmental disabilities
  • Collaborate with healthcare providers and other professionals to ensure comprehensive care

Requirements:

  • Bachelor's degree in Psychology or related field (Master's preferred) with 4 years of experience in Developmental Disabilities
  • 2+ years of direct support experience with individuals with behavior challenges, medical involvement, or psychiatric diagnosis
  • 1+ year of experience coordinating services for individuals with developmental disabilities

Estimated Salary: $49,000 - $52,000 per annum
