- Review and update medication records to ensure accuracy and completeness.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
- Participate in daily team meetings to discuss resident care and address any concerns.
- Assist with medication management, including ordering and stocking medications.
- Maintain accurate and up-to-date records of medication administration and resident care.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- High school diploma or equivalent required.
- Previous experience working with seniors preferred.
- Ability to work effectively in a fast-paced environment.
- Strong communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Ability to work independently and as part of a team.
