KEY RESPONSIBILITIES:
- Guide patients to specialty and other necessary care, addressing barriers such as language, transportation, and financial issues, while ensuring timely and effective referrals.
- Evaluate patient and family preferences for communication methods.
- Provide essential information, including appointment details and health reminders, to patients through various communication channels.
- Adhere to organizational protocols regarding the use of Electronic Medical Records (EMR) in compliance with HIPAA regulations.
- Document all patient interactions accurately and thoroughly in the electronic health record.
- Maintain access to relevant health information systems.
- Utilize health information systems to gather necessary data for care coordination and keep patient records updated.
- Collaborate with community programs and internal departments to assist patients effectively.
- Process prior authorizations and insurance referrals as needed.
- Monitor patient care needs proactively and collaborate with Health Information Technology.
- Collect and analyze data related to healthcare coordination.
Qualifications:
- High school diploma or GED.
- Minimum one year of experience in the medical or health insurance sector.
- At least six months of experience with Microsoft Office applications.
- At least six months of experience with electronic medical record systems.
