We are seeking a skilled Licensed Practical Nurse to join our team at DaVita, a leading provider of dialysis services. As a LPN, you will play a vital role in delivering high-quality patient care in an outpatient dialysis setting.

Key Responsibilities
  • Provide compassionate and empathetic care to patients with end-stage renal disease or chronic kidney disease
  • Collaborate with the healthcare team to develop and implement individualized treatment plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and report any changes or concerns to the healthcare team
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Requirements
  • Current LPN-LVN license in the state of practice
  • CPR certification
  • IV certification (if required by state regulations)
  • High school diploma or equivalent
  • Completed training program approved by the medical director and the governing body
