Job Description
We are seeking an experienced LPN Homecare Nurse to join our team as a Weekend Homecare Clinical Caregiver. In this role, you will provide individualized attention and support to our patients in their homes, helping them manage their health effectively and promoting independence and empowerment. You will work closely with our multidisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient.

Required Skills and Qualifications
To be successful in this role, you will need:
• A current LPN license in good standing
• At least 2 years of experience in home healthcare or a related field
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
